1ヶ月で4名の患者さんを見ました。
(仮)潰瘍性大腸炎・尿路感染・川崎病・生後の呼吸困難
などです。(個人情報保護のため、プライバシーには十分配慮を行なっております。)
小児科は縦横無尽に医学がわかってないと務まりませんね。
教授諮問は計3回ありました。
厳しい先生の諮問は2回です。
「知識をもっと深くしないとあかん。なぜそうなるかまで理解してない」と言われました。
ポリ班のみんなも、わけのわからない症例でもなんやかんや上手く諮問を乗り越えてました。
よくやるなあ。
学んだこと
・生後2ヶ月から5種混合ワクチン(ジフテリア、百日咳、破傷風、ポリオ、Hib)
・潰瘍性大腸炎は絶食治療、タクロリスム
・尿路感染を起こす奇形：膀胱尿管逆流症、水腎炎
・川崎病の症状：発熱、眼球結膜充血、いちご舌、発疹、手足の腫れ(硬性浮腫とか)、頚部リンパ節腫れ＋冠動脈疾患があれば確定診断
・川崎病に対してガンマグロブリン大量療法を行う。対処療法でアスピリン。
ガンマグロブリン大量療法ってなかなか思いつきそうもない治療法ですよね。
例えが変ですが、1/10の異変が起きて「真っ赤になった水」を想像してください。これが川崎病です。大量ガンマグロブリン療法は、この「赤い水」に「大量の透明な水」を入れて薄める治療というわけです。
「大量の水」は最終的に肝臓で代謝されます。
また、群馬スコアというので、大量ガンマグロブリン療法が有効かどうかがわかるそうです。有効でないとわかったら、ステロイドパルスを行い、大量ガンマグロブリンが効くように改造しちゃうんですって。
現代医療はやっぱすごいわ。
親との関わり
小児科は親と関わります。他はそんな科ないです。
親は泊まり込みで小児科病棟にいらっしゃいます。
いろんな親がいるんだなあ、と思いました。
見た目がいかつい人でも、親の姿で見ると印象がまる替わりです。
そんなもんなんですね。
「親」という肩書きを持つと社会的評価もずっと変わるよなあって思いました。
親の前で問診したりするので、緊張はします。
頑張りましょう。
出産を見た感想
最初は恐れ多いものを見たというのが素直な感想。
血がドボドボ出ますからね。
昔は、出産現場は男子立ち入るべからずだったそうです。
それほど過酷なものなのでしょう。
助産師さん看護師さん医師がつきっきりで出産の手助けします。
大体1〜2時間で出産が終わります。
ある説によれば、赤ちゃんが膣から出てくる瞬間に魂が宿るのだそうです。
不思議な光景だなあと思いながら見てました。
あと、助産師さんや看護師の方を見ていると、
やっぱこういう仕事は女性にしかできないよな、って思います。
もし小児科・産婦人科が男だけだったら違和感ありありですよ。
女性にしかできない声掛けとか安心感とかあるよなって思いますね。
僕の正直な感想です。
復活：京都アニメーション
アニメ映画がやってたので見てきました。
京アニは僕の青春ですからねー！
また良い作品作ってくれました。
キャラがかわいい！
こっちも良いです。中毒性あります。
ただ若さを表現したわけでもなく、
そして色恋だけで終わるベタな作品でもなく、
自己表現を思い通りできない思春期の人間模様を表現していて、
とうに封印したはずの自分らの過去と重なり、
なんていうか、「いとをかし」な作品です。
ボクシングジムに行ってみた
良いですね！通い始めようと思います。
地元の人と交流できる良い機会です。
この間はおじさんにボコボコにされました。
おばさんもおじさんをボコボコにしてたり、見ていて楽しいです。
「闘う」のは楽しいですね。
何よりもストレス解消になります。
アドレナリンでっぱなしです。テストステロンでっぱなしです。
ボクシングに通う人たちも、イケメンばかりに思いました。
「闘う」人はこう、病院の人たちとはまた変わった雰囲気があります。
僕が行ったところは道場の形をとっていて、ありがたいことに通い放題です。
9時5時で学校との往復の中に、ボクシングを取り入れるのも良いんじゃないでしょうか。

My mame is naoya nishimaki I'm a medical student of fujita health univercity.
In my education volunteer experiance, I learn how to communicate with students who came from differernt countury.
I want to be a Reconstructive Surgery.So I want to participate in Reconstractive Surgery during CME.
In this program,I want to know different helthcare systems and learn how to built trust with people how have different background.
Disscussion of your medical experience as a medical student and your areas of interest
What is most impressive experience as a medical student is a patient who dameged by traffic accident and hospitalized in Reconstractive Surgery.
She was dameged her face and I thought she is miserable.
But Reconstructive surgeons tried their best and treat her.
Finally, she compretery recover and her face is beautiful again.
she was greateful to the surgeons.I was impressed.
I aim to be be such surgents.
Questions about why you want to be participate in CME abord and your preffered disination.
I'm sure parpeciting in CME abord will curtivat the way of dealing with people from various backgrounds.
Let me talk about my Education volunteer experionce.
My volunteer activity is teaching foreign students Japanese education.
I learn the fact that it is difficult for foreign students to be given educational oppotunities and they are easy to turn to delinquency.
So I'm working with the intention of reducing such people
through my activity.
When I bocome a doctor,I think we will see a lot of foreign patients.
I think it is important to create a system to accepe such forign people to co-exist.
Therefore,through this CME experience,I would like to create a foundation which I can deepen my understanding with people from different cultures.
Taiwan is similer to Japan and a lot of Taiwanees is in Japan,
so we will see them in the future.
Also,Taiwan have a lot of connection with Japan and I want to experience different medicsl system and broaden my horizons
in order to become a doutor who can be active in Asian countries.
Alcohol and cancer risk
About 100,000 caces of cancer each year in united states are due to alchol consumption
and about 20,000 death isn't a small in proble.
mayo clinic says Alchol is the third leading preventable cause of cancer in the US
Behind of tabacco use and obisety.
The risk starts with actually relatively low intakes of alchol over a long period of time
assuming it once in a while probably isn't going to increase the risk of that match
but there are many people who consume that regulary and there are many people who assume
that there health benefits on the heart and there benefits probably aren't as strong as what used to believe
he says when drinking alchol is combined with tabacco ues the risk of cancer increases significantly
Studies have suggest the optimal amount of alchol for people less than 50 is zero we need to dial back
I think a little bit on our alcohol consumption be aware of it an occasional drink of alchol is not going to increase
long-term risk but we need to be aware of it keep a lid on our consumption.
